Notes:

1. The Company will reimburse the total eligible expenses incurred for insured benefits (4) to (14) accumulated per policy year, in excess of the deductible amount.

2. Retirement option allows the policy owner to convert the RM20,000 deductible plan to a RM5,000 deductible plan when the Life Assured attains age 60 years next birthday on the Policy Anniversary without further underwriting.

3. The list of conditions is subject to the Company's review and may be extended to include additional condition(s) from time to time.
